More than 600,000 bottles of a commonly prescribed blood pressure medication were recalled in the United States after a manufacturing issue raised concerns about the source of one of its key ingredients.
The medication involved is ramipril, an ACE inhibitor used to treat high blood pressure and heart failure and, in some cases, to reduce cardiovascular risks following a heart attack. In late 2024, Lupin Pharmaceuticals initiated a nationwide voluntary recall covering more than 600,000 bottles of ramipril in several strengths.
Why Was Ramipril Recalled?
The recall centered on the drug’s active pharmaceutical ingredient, or API.
According to recall information, Lupin was notified by the API manufacturer that a key starting material used to produce the ingredient had been obtained from an unapproved vendor. That constituted a current good manufacturing practice (CGMP) deviation and prompted the company to recall affected lots.
Importantly, the California Board of Pharmacy’s recall notice said the recalled drug products, the API and the key starting material all met specifications. Because of that, the potential health hazard could not be predicted rather than being identified as a confirmed contamination event.
The FDA classified the recall as Class II. That designation is generally used when exposure to a recalled product may cause temporary or medically reversible health consequences, or when the likelihood of serious harm is considered remote.
How Many Bottles Were Affected?
The recall covered ramipril capsules in 2.5 mg, 5 mg and 10 mg strengths.
The three recall actions totaled approximately 616,500 bottles:
- 112,770 bottles of the 2.5 mg strength
- 146,322 bottles of the 5 mg strength
- 357,414 bottles of the 10 mg strength
The affected products were manufactured by Lupin Limited in Goa, India, for Lupin Pharmaceuticals in Baltimore, Maryland. The products had been distributed nationwide through wholesalers and distributors.
What Does This Mean for Patients?
A recall involving a commonly used prescription medication can understandably cause concern, particularly for people who take it every day to control their blood pressure.
However, patients should not abruptly stop taking ramipril simply because they have heard about the recall. Suddenly discontinuing blood-pressure medication without medical guidance can itself create health risks.
Instead, patients who take ramipril should check their prescription bottle against the recall information and speak with their pharmacist or health-care provider if they believe they have an affected lot. A pharmacist can determine whether a patient’s particular bottle is included and, if necessary, help arrange a replacement.
The recall applied to specific lots and package sizes rather than every bottle of ramipril on the market.
The FDA’s Role
The wording that the recall happened because of an “FDA slip-up” can be misleading.
The available recall information indicates that Lupin initiated the voluntary recall after receiving notification concerning the source of the API’s starting material. The FDA subsequently posted the recall information as part of its drug-safety and enforcement system.
The FDA maintains a searchable database of drug recalls so consumers, health professionals and industry members can review products subject to regulatory action.
A Reminder About Drug Recalls
Drug recalls do not necessarily mean that a medication has been proven dangerous or that everyone who took it will become ill.
